Job Description:
A company is looking for a Virtual Mental Health Resource and
Discharge Planner.
Key Responsibilities
Meet with patients to discuss structure and time management and
develop individualized resource plans
Establish relationships with local resources and manage referral
lists for patient support
Collaborate with the treatment team to prepare patients for
successful discharge and aftercare planning
Required Qualifications
Master's degree in counseling, psychology, social work, or a
related field
Knowledge of motivational interviewing techniques
Experience with Cognitive Behavior Therapy (CBT) and Dialectical
Behavior Therapy (DBT) is preferred
Ability to manage multiple patients with varying needs
Flexibility and creativity in problem-solving and developing
resource plans
