Job Description:
A company is looking for a Patient Scheduler for Medication
Therapy Management (MTM).
Key Responsibilities
Perform outreach to targeted members and schedule telephone-based
medication reviews
Document required data elements using a software platform to assist
in care documentation
Coordinate with partner teams for data entry audits and regulatory
reporting
Required Qualifications
1+ years of related experience, including work, education, and
military experience
1 year experience working with members/clients/patients on the
phone
Proficient in using computer applications
Organized and efficient in task completion and meeting
deadlines
Experience in a healthcare-related setting is preferred
