Job Description:
A company is looking for a Remote Case Manager, RN to advocate
for patients and enhance their quality of care.
Key Responsibilities:
Collaborate with a healthcare team to monitor utilization patterns
and facilitate appropriate healthcare delivery
Conduct comprehensive assessments of members' health status,
including medical records and clinical history
Work with healthcare providers and family members to develop and
coordinate treatment and discharge plans
Required Qualifications:
North Carolina or compact RN license required
Prior telephonic case management experience is a plus
3+ years of inpatient care experience
Critical care experience, including med-surg and ICU
Must reside in the EST time zone
